How Is Breast Augmentation Performed?

Breast augmentation is performed under general anesthesia and takes less than a hour to perform; however, this length can increase if you choose to combine your breast augmentation with any other surgical procedures.

Breast Implant Options

Breast implants consist of a silicone shell filled either with saline or silicone. When placed under the breast they enhance breast shape and volume. Implants come in various shapes and sizes. You will have the opportunity to discuss your implant selection with Dr. Porshinsky during your consultation.

  • Saline implants are filled with a sterile saline solution. This option requires smaller incisions since the implant is not filled until after it is placed in the breast pocket. While saline implants can provide subtle or dramatic augmentation, some women feel as though they do not look or feel as natural as silicone implants.
  • Silicone implants are filled with a cohesive silicone gel that is meant to mimic the look and feel of natural breast tissue. There are many types of silicone implants with different levels of cohesiveness. The right silicone implant for you will be determined during your consultation based on what you hope to achieve with your surgery.

Breast Augmentation Incision

Dr. Porshinsky prefers to perform breast augmentation surgery using the inframammary incision. The inframammary fold is an important anatomical landmark and is the sharp breast crease beneath the breast (where the bottom of the breast meets the ribcage).

During breast augmentation, an incision is made across this fold, and the implant is inserted into a created breast pocket above or below the pectoralis muscle. When healed, the resulting scar is nearly invisible and will be hidden within the natural crease beneath the breast.

While it is possible to use other incision methods for breast implant placement, including incisions made around the areola or within the armpit, Dr. Porshinsky finds that the best results (and fewer risks) come from the inframammary incision.

Breast Implant Placement

Breast implants are placed beneath the pectoral muscle (submuscular) or above the muscles (subglandular or subfascial). The proper placement will depend on the results you are looking to achieve and the amount of natural tissue you have.

If you have little natural breast tissue and fat, placing the implants in the submuscular placement allows for better implant coverage, reducing the risk of implant rippling (where the edges of the implant can show through the skin) and implant contracture over the long term.

Breast Augmentation Combinations

Breast augmentation is commonly performed together with breast lift surgery to tighten the surrounding tissues and elevate the newly enlarged breasts. This combination may be recommended if you choose breast implant surgery after one or more pregnancies or substantial weight loss.

Some women even choose to incorporate breast enhancement surgery into a more significant body contouring procedure, such as a Mommy Makeover. A Mommy Makeover combines breast surgery with a tummy tuck and/or liposuction to target the areas most affected by pregnancy or weight loss.

Breast Augmentation Using Fat Grafting

Women looking for subtle breast enlargement or increased soft tissue coverage around an implant can use fat grafting to augment the breasts. Fat grafting to the breast is similar to Brazilian Butt Lift surgery in that it uses fat that has been removed from your body to increase the volume of your breasts.

This option is not a replacement for breast implants and cannot achieve the same volume as implants; however, it can provide natural-looking fullness. Fat grafting can also be combined with breast implants to achieve a more natural shape. This technique also helps with asymmetries. Dr. Porshinsky can sculpt an ideal breast shape for you.

How Long Is Breast Augmentation Recovery?

Breast augmentation recovery is relatively simple and straightforward compared to other plastic surgery procedures.

You will be wrapped in a compression bra following the operation, which you will be required to wear around the clock during your recovery. This garment will help reduce bruising and swelling. You can expect some discomfort once the anesthesia wears off; however, prescription or over-the-counter pain medication can be taken.

Most patients can return to work and their normal day-to-day activities within a few days; however, strenuous upper-body exercises must be avoided for several weeks or until you have been cleared to resume these by Dr. Porshinsky.

When Will I See My Breast Augmentation Results?

While you will notice fuller breasts immediately after your breast augmentation, swelling and bruising will conceal the final results. Additionally, it is common for breast implants to appear higher and feel firmer in the chest initially. It takes time for the implants to settle into place and feel soft and natural. This is known as the drop and fluff stage, which can take several weeks.

Most patients realize their final results within a month. The breast implants themselves will not be affected by weight fluctuations or pregnancy, but the surrounding breast tissue may, meaning that your enhanced breasts may change shape over time. Breast revision procedures are available to address any unwanted changes.
